Realise provides tailored support for people who may be living with their family, living in their own accommodation or to those who wish to progress to more independent living.

Realise is recruiting for a React Support Worker, supporting on their crisis team.

Join our Realise Team, part of the Somerset Care Group, in Somerset. Our tailored service provides support to people within their own homes, out and about, with personal care, hobbies, or companionship.

To apply for this role, you will need to ensure you have: 

  • A full valid driving licence and your own vehicle.  
  • Knowledge of the Somerset area and able to commute around Somerset from your home location. 
  • Right to work in the UK (unfortunately we are not able to offer sponsorship). 
  • Available to work alternate weekends
  • 2 Years experience within challenging settings
  • Level 2 – Adult Social Care

A REACT Support Worker is our most essential role and involves:

  • As a valued member of our team, REACT Support Workers are key to helping us provide the support and care our clients deserve. The React team support individuals who are in crisis across Somerset, working closely with the rapid intervention team to prevent hospital admissions.
  • REACT Support Workers do more than a job, they change a life. You would be part of a team that provides high-quality, person-centred care, tailored to the needs of each individual.
  • You’ll be completing assessments and implement protocols and strategies to reduce behaviours that challenge and to enable individuals to live a fulfilled and meaningful life.
